US experts urge approval of first AIDS prevention pill

WASHINGTON (AFP) – US health advisers on Thursday urged regulators to approve Truvada, made by Gilead Sciences, as the first preventive pill against HIV/AIDS instead of just a treatment for infected people.

The favorable vote came after clinical trials showed Truvada could lower the risk of HIV in gay men by 44% to 73%, and was hailed by some AIDS advocates as a potent new tool against human immunodeficiency virus.

However, many concerns were raised during a marathon 11-hour panel meeting in which about three dozen health care providers warned that the pill could boost risky behaviors and possibly lead to a drug-resistant strain of HIV.

The Food and Drug Administration is not bound by the recommendations of its expert panel, but usually follows the advice. A final decision by the FDA is expected by June 15.

Mitchell Warren, executive director of HIV prevention group AVAC, said after the vote that pre-exposure prophylaxis (PrEP), or the method of taking a drug ahead of potential exposure to HIV, “while not a panacea, will be an essential additional part to the world’s success in ending AIDS.”

“For the millions of men and women who remain at risk for HIV worldwide, each new HIV prevention option offers additional hope,” he added.

The drug, made by the California-based Gilead Sciences, is currently available as a treatment for people with HIV in combination with other anti-retroviral drugs, and received FDA approval in 2004.

The panel’s nod came in response to the pharmaceutical company’s request for a supplemental new drug application to market it for prevention purposes.

The Antiviral Drugs Advisory Committee voted for the drug as a preventive measure for three groups: 19-3 in favor for men who have sex with men, 19-2 with one abstention for couples in which a partner is HIV positive and 12-8 with two abstentions for other at-risk groups.

Gay men account for more than half of the 56,000 new HIV cases in the United States each year, according to the Centers for Disease Control and Prevention (CDC).

But critics noted that the pill is costly — up to $14,000 per year — and could offer a false sense of protection, leading to a spike in unsafe sex and a new surge in AIDS cases.

“We need to slow down. I care too much about my community not to speak my concerns,” said Joey Terrill, advocacy manager at the AIDS Healthcare Foundation, which campaigned against the drug’s approval for PrEP.

There also remains some controversy about who would benefit from the treatment, as trials in women have shown feeble results, possibly due to poor adherence to the regimen.

“I am concerned about the potential for development of resistance,” said Roxanne Cox-Iyamu, a doctor who spoke at the panel’s meeting.

“I am concerned as a black woman that we don’t have enough data that this actually works in women.”

Nurse Karen Haughey said Truvada will not work because “it is not in our nature to always do as human beings what we are told 100 percent of the time.”

She also said Truvada’s main side effects — diarrhea and risk of kidney failure — were a major deterrent.

The main set of data considered came from the iPrEx HIV Prevention Study, carried out from July 2007 to December 2009 in six countries — Brazil, Ecuador, Peru, South Africa, Thailand and the United States.

The study was conducted among 2,499 men who were sexually active with other men but were not infected with the virus that causes AIDS.

Participants were selected at random to take a daily dose of Truvada — a combination of 200 milligrams of emtricitabine and 300 milligrams of tenofovir disoproxil fumarate — or a placebo.

Those in the study who took the drug regularly had almost 73 percent fewer infections. Across the entire study, including those who had not been as diligent in taking Truvada, there were 44 percent fewer infections than in those who took a placebo.

After publication in 2010 in the New England Journal of Medicine, some experts hailed the results as game-changing and the first demonstration that an already-approved oral drug could decrease the likelihood of HIV infections.

Joseph McGowan, medical director of the Center for AIDS Research and Treatment at North Shore University Hospital in New York, said the CDC was expected to soon issue guidance for health professionals who may prescribe the drug.

“I don’t see it as something that would be useful to the general public but to certain people who are particularly high risk, there may be some benefit,” he said. - Agence France-Presse

HIV Registry: March 2012

According to the March 2012 report of the National Epidemiology Center, 313 individuals were confirmed HIV Ab sero-positive by the STD/AIDS Cooperative Central Laboratory. The figure is 82 percent higher of the same period last year. Moreover, the data has been the highest number of cases ever reported to the registry.

Out of the 313 cases, 17 were reported to be AIDS cases (16 male and 1 female). All cases were acquired through sexual contact. Three deaths due to AIDS were also reported for the month.

Aside from the information above, here are some of the highlights of the report:

  • Most of the cases (93%) were males
  • Median age was 29 years (age range: 2 -81 years)
  • The 20-29 year age-group had the most number of cases at 52 percent
  • 146 of the 313 cases were from the National Capital Region
  • Reported mode of transmission were sexual contact (280), needle sharing among injecting drug users (31), and mother to child transmission (2).

HIV Registry: December 2011

The latest report of the National Epidemiology Center revealed that there were 268 new HIV Ab Sero-positive  individuals confirmed by the STD/AIDS Cooperative Central Laboratory. This is a 54 percent increase compared from the same month in 2010. With this additional data from December, the total number of cases for 2011 reached 2,349.

Out of the 268 cases, nine were reported to be AIDS cases (seven males and two females). All of the cases were acquired through sexual contact. A death due to AIDS was also reported for this month.

Aside from the information above, here are some of the highlights of the report:

  • 94 percent of cases were males
  • The age range was 16 to 59 years and the 20 to 29 year group had the most number of cases
  • 128 of the cases came from Metro Manila
  • The reported modes of transmission were sexual contact (238) and needle sharing among  injecting drug  users (30)
  • 8 percent of the cases were OFWs

According to the latest report of the National Epidemiology Center, 212 new HIV Ab sero-positive individuals were confirmed by the STD/AIDS Cooperative Central Laboratory for the month of November. The figure is 89 percent higher from the same period last year.

Of the 212 cases, three were reported to be AIDS cases. These were two males and a female. All acquired the infection through sexual contact (1 heterosexual and 2 homosexual). There was also reported death from AIDS for the month, a 26 year old female.

Based on the November 2011 report, here are the other observations:

  • 94 percent of the cases were males
  • The age range was between 17 to 73 years and the media age was 28 years
  • 124 cases or 58 percent of it were from the National Capital Region
  • The reported mode of transmission was all sexual contact with males having sex with other males as the most predominant at 85 percent
  • 99 percent of the cases were still asymptomatic at the time of reporting
  • 12 percent (25) of the 212 were OFWs

The October 2011 report of the Department of Health’s National Epidemiology Center revealed that 200 new HIV Ab sero-positive individuals were confirmed by the STD/AIDS Cooperative Central Laboratory. The number is 92 percent higher compared to the same period last year.

Out of the 200 HIV cases, only one was reported to be an AIDS case.  There were also no reported deaths.

Aside from these , here are some of the observations made for October:

  • 97 percent of the reported cases were males
  • The 20-29 age group had the most number of cases
  • 108 or 54 percent of the reported cases were from the National Capital Region
  • Reported modes of transmissions were sexual contact (183) and needle sharing among injecting drug users (17)
  • The predominant type of sexual transmission was males having sex with males
  • 21 of the 200 reported cases were OFWs (19 males and 2 females)

According to the September 2011 report of the National Epidemiology Center, 253 new HIV Ab sero-positive individuals were confirmed by the STD/AIDS Cooperative Central Laboratory. The figure is 65 percent higher compared to the same period last year.

Here are some of the other reported observations:

  • 95 percent of the cases were males.
  • The 20 – 29 year age group had the most number of cases at 57 percent.
  • 135 cases came from the National Capital Region.
  • Reported mode of transmission were sexual contact and needle sharing among injecting drug users.
  • Males having sex males was the predominant type of sexual transmission at 83 percent.
  • Sixteen of the reported cases were OFWs, acquired through sexual contact.
Of the 253 cases, five (all males) were reported to be as AIDS cases.  Four cases were acquired through sexual contact [homosexual (3) and bisexual (1)] and one case from needle sharing. There were also two reported deaths for September, again both males aged 40 and 49 years old.

According to the report released by the National Epidemiology Center, 196 individuals were confirmed by the STD/AIDS Cooperative Central Laboratory as HIV Ab sero-positive for the month of August. The figure is 81 percent higher compared to the same period last year.

Also, out of the 196 reported cases, seven were reported to be AIDS cases. There were also four reported deaths due to AIDS.

Here were the observations made for the month of August:

  1. Most of the cases were males at 93 percent.
  2. The 20-29 age group had the most number of cases.
  3. Fifty-five percent of the reported cases came from the National Capital Region.
  4. Reported modes of transmission were (1) sexual contact, (2) needle sharing among drug users and (3) mother-to-child transmission.
  5. Males having sex with other males were the predominant type of sexual transmission.
  6. Twenty-three individuals reported to be HIV positive were Overseas Filipino Workers.

Where can I get an HIV Anti-Body Test?

We’d like to thank our friend Lean Emmanuel Aseron, who is an AIDS Society of the Philippines Online Peer Educator for giving us an updated list of accredited Testing Sites where you can get an HIV anti-body test.

Please remember that under Philippine Law (Republic Act 8504), you cannot be mandated to take an HIV anti-body test. The law also upholds your right to confidentiality (you may choose to use an alias or a number code in place of your name) and your right to have pre and post counseling.

Take the test. Know your status.  Spread the love, not the virus.

HIV Registry: July 2011

In the recent report released by the National Epidemiology Center of the Department of Health, 204 individuals were found HIV Ab sero-positive. The number is 56 percent higher compared to July of last year.

 

From the report, the following observations were noted:

1. Three of the 204 reported HIV positive cases were AIDS and all was acquired through sexual contact.
2. Males comprised 90 percent of the cases.
3. Age range for the reported cases were 17 to 55 years old. It was the 20-29 year age group that had the most number of cases at 63 percent.
4. 107 of the 204 reported cases were from the National Capital Region.
5. Modes of transmission were sexual contact (193) and needle sharing among injecting drug users (11). Males having sex with males were the predominant type of sexual transmission at 81 percent.
6. Two deaths were reported for July.
